Dr. Centeno's office has relocated to 169 Dayton Street, Ridgewood, NJ, as of January 1, 2009.  Office hours are by appointment.  To schedule an appointment, please call (201) 925-9700 or send email to drcenteno@optonline.net.

Insurance

At this time, Dr. Centeno is not an in-network provider with any insurance plan, except for Medicare.  Individuals are expected to pay at the time of service and a bill will be given with sufficient information to submit to insurance for reimbursement.

Take Route 17 to the Ridgewood Avenue exit (towards Ridgewood).  Follow Ridgewood Avenue and make a left onto South Maple Avenue.  Take the first Right onto Dayton Street.  169 Dayton Street is on the right.  Please park in the parking lot two lots up from the building.  It is a parking lot with no sign.
