Dr. Centeno's office is located at 115 Pine Street in Ridgewood, NJ (near the Hohokus border). Office hours are by appointment. To schedule an appointment, please call (201) 925-9700 or send email to drcenteno@optonline.net.

Insurance

At this time, Dr. Centeno is not an in-network provider with any insurance plan, except for Medicare.  Individuals are expected to pay at the time of service and a bill will be given with sufficient information to submit to insurance for reimbursement.

Office Directions

Take Route 17 North, get off at Linwood Avenue exit (towards Ridgewood). Follow Linwood Avenue and make a Right onto North Maple Avenue. Turn Left on Pine Street (across from the Ford Auto Dealership). Make a Right into the second driveway on Right. The building has its own parking lot. Dr. Centeno's office is on the third floor (no elevator access).
